HOW TO ENROLL YOUR CHILD: YOUR REGISTRATION IS NOT COMPLETE AND YOUR CHILD'S SPOT WILL NOT BE HELD UNTIL THE FIRST WEEKS TUITION AND REGISTRATION FEE IS RECEIVED BY OUR OFFICE. 1. Complete a Registration Form for each child being enrolled. 2. Attach a $35 registration fee and first week�s tuition with each Registration Form 3. Mail 1st weeks tuition and registraton fee to: Four Seasons Child Care, 706 Lehmann Way, Somerset WI 54025. Once your child�s registration is received by Four Seasons Child Care, you will be asked to fill-out additional paperwork (health/immunization form, emergency contact form, special intake form for children under two years, etc.). AMOUNT DUE AT TIME OF REGISTRATION: $35 Registration Fee, plus first week�s tuition per child (refer to Rate Sheet) I UNDERSTAND THAT: At the time of my child�s registration, I will pay a $35 registration fee, plus first week�s tuition. In order for my child to be placed on Four Seasons Child Care�s waiting list, I must pay the registration fee and first week�s tuition for my child. There are no refunds for tuition or registration payments (see exception below). All coupons & promotions, in order to be valid, must be presented prior to enrolling your child. If my child is placed on an enrollment waiting list at the time his/her Registration Form is submitted, I will receive prompt notice of the next available opening. I will then will have two weeks to decide if my child will start immediately or lose his/her position on the waiting list. If I choose to give up my child�s waiting list position, I will receive full refund of my already-paid registration fee and first week�s tuition. FOR EXAMPLE, if your child�s registered start date for an age-specific classroom is September 1st, and that class indicates full capacity on August 1st, AND a potential student has requested to start immediately in that classroom, Four Seasons Child Care will contact you to offer you the option to start your child immediately; and if that�s not feasible for your child, s/he will lose his/her waiting list position in order to allow the potential student to start immediately. A full two-week written notice is required from the parents prior to withdrawing their child from Four Seasons Child Care. If the parents decide that their child�s final day will be prior to the full two-week notice date, the parents are still responsible for paying tuition for any unattended days within the full two-week period.
